  1. Power of attorney instructions (text of the power of attorney according to the applicant’s needs, preferably drafted by a lawyer, in electronic format WORD).
  2. Copy of the valid Bolivian or foreign passport or identity card of the power of attorney.
  3. Copy of the valid passport or identity card of the attorney-in-fact.
  4. Complete data of the attorney-in-fact and power of attorney (full name, marital status, profession or occupation, identity card, place of issue and address).
